Gender affirming care, like all of medicine, should be individualized to the goals of each patient seeking care. In order to help me get really clear on those goals, I use a yes/no/maybe chart. Patients can complete it before a visit and it helps me see their goals at a glance. It not only speeds up care, it improves the quality and individualization of care each patient gets.
